The Cold and Fever Saying Has Been Backwards for Centuries — Medieval Doctors Noticed
At some point in your life, someone sick in your family either got extra food or was told to skip a meal — and whoever made that call cited the same phrase. "Starve a cold, feed a fever." Or maybe it was the other way around. "Feed a cold, starve a fever." Most people who repeat it aren't actually sure which version is correct, and that uncertainty has been baked into the saying for longer than you'd expect.
Here's the part that gets interesting: medieval physicians had a clear, reasoned position on this. The confusion didn't come from them. It came from centuries of retelling, a single famous dictionary entry, and the very human tendency to pass along advice without fully understanding it.
What Medieval Medicine Actually Recommended
To understand the original logic, you have to step into how pre-modern physicians understood illness. Medieval European medicine was built on the humoral theory inherited from ancient Greek physicians — the idea that the body was governed by four fluids: blood, phlegm, yellow bile, and black bile. Health meant keeping these in balance. Disease meant something had disrupted that balance.
Under this framework, a cold — what they called a "cold and moist" condition — was treated by warming and drying the body. Food, particularly warm and hearty food, was considered a tool for generating internal heat. Feeding a cold made logical sense within their system: you were adding fuel to warm a body that had gone too cool and damp.
Fever was the opposite problem. A fever was already a "hot" condition. Physicians believed overloading a feverish body with food would add more heat to a system that was already dangerously warm. The recommended approach was rest, light broths, and reduced intake — not starvation exactly, but a deliberate pulling back.
So the original version of the advice, rendered in plain terms, was: feed a cold, starve a fever.
That's not backwards. That's the medieval original — and it has a certain internal logic, even if the underlying theory of humors turned out to be wrong.
How It Got Flipped
The first written record of the phrase in English appears in a 1574 dictionary by John Withals, which included the line: "Fasting is a great remedie of feuer." The cold half of the equation was implied by contrast. Feed the cold, fast through the fever.
For a while, the saying circulated in roughly that form. Then something happened that language tends to do to proverbs: people started interpreting it conditionally rather than literally.
By the 19th century, a popular misreading had taken hold. Some writers began interpreting the phrase as a warning rather than a prescription — as in, if you feed a cold, you will have to starve a fever later. The logic being that overindulging during a cold would cause a fever to follow, requiring fasting as a correction.
This interpretation flipped the cause-and-effect relationship entirely. Instead of two separate instructions for two separate conditions, the saying became a cautionary chain of events. And once that reading spread, the phrase became genuinely ambiguous. People started repeating both versions, unsure which was the "real" one, and the confusion became self-perpetuating.
What Modern Research Actually Found
Here's where it gets more complicated: contemporary science has partially rehabilitated both sides of the argument, though not for the reasons medieval physicians would have expected.
A study published in the journal Clinical Nutrition in 2002 found that eating a meal stimulated immune responses associated with fighting viral infections — the kind that cause colds. Fasting, meanwhile, appeared to activate different immune pathways more relevant to bacterial infections that often come with fever. The researchers were careful not to overstate the finding, but the rough implication was that feeding during a cold and reducing intake during certain fevers might actually have some biological basis.
More recent research has complicated even that picture. A 2016 study from Yale suggested that caloric restriction during bacterial infection (fever) could be harmful, not helpful. The science, in short, is still unsettled — and "eat nutritious food and rest when you're sick" remains the most defensible general advice.
What modern medicine does agree on is that nutrition matters during illness. Severely restricting food while sick doesn't help your immune system do its job. Hydration, adequate calories, and rest are consistently supported. The idea of dramatically "starving" anything is not.
Why the Saying Kept Going
Proverbs survive because they're memorable, not because they're accurate. They compress complicated ideas into short phrases that feel authoritative — and once a phrase enters the cultural bloodstream, it develops a kind of immunity to correction.
The cold-and-fever saying also survived because it sounds like it came from a doctor. It has the cadence of clinical advice. That tone alone gives it staying power, even when the content has drifted far from its origins.
There's also the comfort factor. When someone is sick, people want to do something. Following a simple rule — even a garbled one — feels better than saying "we're not entirely sure."
The Short Version
The original phrase said to feed a cold and ease up during a fever. It got inverted, reinterpreted, and eventually confused to the point where most people aren't sure which way it goes. Modern science doesn't fully validate either version — but it does suggest that eating reasonably, staying hydrated, and resting is good advice regardless of whether you have a cold or a fever.
The medieval doctors weren't entirely wrong. They just didn't have the benefit of knowing their advice would survive seven centuries of telephone.
